Due to a facility safety concern, there will be no school tomorrow, Wednesday, January 23 and Thursday, January 24, and no one should enter the school building during this time. 

These will be cyber days for Upper and Middle schools.

An update will be sent Thursday afternoon.

Athletic Registration and Forms


  • PIAA physical form: Students who will play on our MS or US teams, you must turn in this form. You fill out the first five pages; your doctor fills out Page 6.

  • MS-US Recertification form: Once a MS or US student-athlete completes his first season of play, he must submit  this to participate in any future sport that school year. If the recertification form indicates any injuries during the first season, this physicians' recertification form must be turned in as well.  
  • ImPACT concussion test: Seventh, ninth and 11th graders who play contact sports need to take the ImPACT concussion test. Be sure to allow pop-ups in your browsers settings, then click on the link. When asked for the Customer Code enter CCBA6A64F8. Athletics Eligibility

Foreign Exchange students 
Foreign Exchange Student/International Student Eligibility Agreement: Students must submit this form to the Athletic Director, along with a copy of their PIAA physical and J-1 visa.

Foreign exchange students must be working with a program that is on the CSIET approved program list to apply for athletic eligibility. 
Transfer students
Students must submit this form to the Athletic Director.  Parents/students should fill out sections 2 and 3.  Once this form is submitted, we will obtain the signature from the principal of the school you are transferring from, and we'll submit it to the District 3 Executive Director for approval.
Note:  Each transfer case is unique.  Many factors (when the student began participating in school sports, if the student has repeated or is repeating a grade, the student's age, etc) may affect a student's athletic eligibility.   We will work with each family on a case by case basis to be sure we are abiding by all PIAA rules and regulations.
