Student athletes in grades 7-12 must complete the PIAA physical form (dated no earlier than June 1) and the LCDS medical form to be elibigile for participation in athletics.
Students in grades 2-6 must complete the cardiac arrest form and the LCDS medical form to be elibigile for participation in athletics.
Once a student-athlete completes his first season of play, he must submit this recertification form to participate in the next season. If the recertification form indicates any injuries during the first season, this physicians' recertification form must be turned in as well.
